PS2 - Ditch the Diaper: Eliminating CAUTI rates in the hospital
DescriptionAs the 5th leading healthcare-associated infection, Catheter Associated Urinary Tract Infections (CAUTI) contribute to up to 13,000 deaths annually. Complications include patient discomfort, prolonged hospital stay, increased mortality, and increased cost. The average CAUTI cost $13,793 per patient for a cumulative $340 million in the US annually (2017). The Cincinnati Children's Hospital's Pediatric Intensive Care Unit (PICU) implemented an interdisciplinary quality improvement project with the goal of eliminating CAUTI.

After a retrospective review of previous CAUTI events, literature search, and best practices comparison against other hospitals, the identified intervention was to trial removing diapers from all patients with indwelling catheters. This presentation will review the methods and steps taken to integrate non-diapering practices into the CAUTI prevention bundle including approval by leadership, spread to other units, and initiation of the trial.

The trial was successful and subsequently spread throughout the entire system. From December 2020 to April 2022, PICU had ZERO CAUTI's. Since then, the CAUTI team and Human Factors team has continued to work together to understand deviance from the process and how to improve adherence to non-diapering. This presentation will include the data from the implementation and spread, and how we continued work to improve the system through the Human Factors lens.
